Medical Records

Your character's medical records should include any and all major medical events (injuries, illnesses, pregnancies, etc.) that your character has experienced in their lifetime. Medical record entries include the diagnosis (the name or type of injury or illness), the treatment (what was done about it), and anything else the doctor may wish to note (things to watch out for in the future), as well as the name of the doctor, and the ship or station the patient was treated at.
